Absence Reporting

We collect information about you when you submit this via our absence reporting forms. Information collected enables Premier Care to effectively manage absence and ensures we have the correct information for your subsequent return to work interview or welfare meeting.

Information includes:

  • Name
  • Name of the manager you reported absence to
  • First day of absence
  • When you reported fit for duty
  • Details regarding contact with GP and advice provided by GP
  • Details about evidence of appointments
  • When you expect to return to work

Information collected enables us to:

  • Ensure we have accurate information for your subsequent return to work or welfare meeting
  • Ensure that you receive effective support from management with regards to absence i.e. we are able to identify reasonable adjustments if relevant
  • Ensure that we can monitor absence levels effectively

Information is kept secure:

All information is stored on a secure, password protected, computer

New Direction Support will not sell or share your information to third parties.

Casual Availability

We collect information about you when you submit this via our casual availability form. Information collected enables Premier Care to ensure that your availability for shifts is taken into account when creating rotas.

Information includes:

  • Name
  • Your availability for work for the month ahead
  • Any other details you provide within the text field section of the form

Information collected enables us to:

  • Avoid you being rota’d for shifts outside of your availability
  • Check your submitted availability if there is a discrepancy
